PT-141, known by its FDA-approved brand name Vyleesi, is a synthetic peptide of the melanocortin family. It's the first non-hormonal medication approved in the U.S. to work directly on brain circuitry involved in sexual desire, rather than acting on blood flow the way most sexual-health medications do.
PT-141 activates melanocortin-4 receptors (MC4R) in the central nervous system, engaging dopamine-linked pathways in the hypothalamus that are involved in sexual arousal. Phase 3 clinical trials (RECONNECT)
Two identical, randomized, double-blind, placebo-controlled Phase 3 trials enrolling more than 1,200 premenopausal women found significant improvement in sexual desire and related distress versus placebo, supporting FDA approval as Vyleesi in June 2019.
FDA-approved safety data
Trial data showed nausea (roughly 40% of patients on first dose, diminishing with continued use), flushing, and headache as the most common side effects; the drug carries cautions for uncontrolled high blood pressure and certain cardiovascular conditions.
Regulatory divergence outside the U.S.
Vyleesi has not received marketing authorization in the EU, Canada, or the UK, reflecting differing regulatory views on the clinical significance of its effect size and on cardiovascular safety signals.

Compounded versions of bremelanotide used off-label have not themselves been FDA-reviewed for safety or efficacy — only the branded Vyleesi product went through the RECONNECT trials and FDA review.
