How it works.

GHRP-2 (pralmorelin) is an early-generation growth hormone secretagogue and the only compound of its kind to ever receive approval from a national drug regulator anywhere in the world — Japan's PMDA approved it in 2004, but strictly as a diagnostic test for growth hormone deficiency, not as a standing therapy.

GHRP-2 activates the ghrelin receptor (GHS-R1a), triggering GH release from the pituitary. Compared with newer, more selective secretagogues, GHRP-2 also meaningfully raises ACTH, cortisol, and prolactin, which is the tradeoff researchers point to when comparing it against options like ipamorelin.

The Evidence

Approved as a diagnostic agent in Japan (2004)

Japan's PMDA approved pralmorelin as a diagnostic test for GH deficiency based on a multicenter randomized trial (N=120) validating a GH-response cutoff, plus an earlier validation study (N=85) — real regulatory approval, but for a one-time diagnostic injection, not ongoing use.

Failed Phase II therapeutic trials for short stature

A separate development track studying GHRP-2 as a growth-promoting therapy for GH-deficient children — including an intranasal formulation — increased endogenous GH secretion but did not translate into clinically significant height gains over 24 weeks, and that program was discontinued.

No FDA approval or U.S. trial program

GHRP-2 has never been approved by the FDA for any use and has no active U.S. clinical trials as of this writing; its only regulatory approval anywhere is Japan's narrow diagnostic indication.
