Sermorelin is the first 29 amino acids of naturally occurring growth-hormone-releasing hormone (GHRH) — the shortest fragment that still fully activates the GHRH receptor on the pituitary gland. It has the longest regulatory track record of any peptide on this page: it was FDA-approved in 1990 for diagnostic use and later as a pediatric growth-hormone-deficiency therapy before its manufacturer discontinued the branded product.
Sermorelin binds the GHRH receptor on pituitary somatotroph cells, prompting the pituitary to release its own growth hormone in the same pulsatile pattern the body uses naturally. Because it works upstream of GH itself, the pituitary's own feedback controls stay intact — a key difference from injecting growth hormone directly.

The Evidence
FDA-approved diagnostic and pediatric therapeutic agent (1990s)
Sermorelin was approved by the FDA in 1990 for diagnostic testing of pituitary GH reserve, and in 1997 (as Geref) for treating idiopathic growth hormone deficiency in children with growth failure — real regulatory history, not a modern marketing claim.
Discontinued for commercial, not safety, reasons
The branded product was voluntarily withdrawn from the U.S. market in 2008 as recombinant human growth hormone came to dominate the pediatric GHD market, making the smaller-effect, twice-daily sermorelin commercially unviable — not because of a safety or efficacy finding.
Therapeutic trials showed smaller effects than direct GH replacement
In children who responded to GHRH stimulation, daily sermorelin produced meaningful but smaller height-velocity gains than direct recombinant GH therapy, with a milder side-effect profile — an honest trade-off, not a superiority claim.
Adult, off-label use lacks the same trial base
The formal clinical trial record concerns pediatric growth hormone deficiency; adult growth-hormone-support use is compounded and prescribed off-label, without an equivalent body of placebo-controlled adult outcome trials.
