How it works.

Ipamorelin is a pentapeptide growth hormone secretagogue developed by Novo Nordisk and derived from the older compound GHRP-1. It's best known for being the first GHRP-family compound shown to stimulate GH release without meaningfully raising cortisol or prolactin, which is why licensed programs favor it over earlier-generation peptides like GHRP-6.

Ipamorelin activates the ghrelin receptor (GHS-R1a) on the pituitary, triggering a single, sharp pulse of GH release that mimics the body's natural pattern. Unlike older GHRPs in its class, it does this with a high degree of receptor selectivity, largely sparing the ACTH/cortisol and prolactin pathways that other secretagogues also activate.

The Evidence

Preclinical selectivity studies (Raun et al.)

In rat and swine studies, ipamorelin released GH with potency comparable to GHRP-6, but — unlike GHRP-6 and GHRP-2 — did not meaningfully raise ACTH or cortisol, even at doses over 200 times its GH-release threshold. This selectivity finding is the core evidence behind ipamorelin's reputation, but it comes from animal models.

Small human PK/PD studies

Two published dose-escalation studies in healthy male volunteers (roughly 8 subjects per dose group) confirmed dose-proportional pharmacokinetics and a short-lived, single GH pulse peaking under an hour after dosing — real human data, but small, short-duration pharmacology studies, not outcome trials.

Discontinued Phase II trial for postoperative ileus

Ipamorelin reached a completed Phase II trial (NCT00672074) for postoperative ileus, supported by rat models showing increased GI motility, but development for that indication was discontinued without regulatory approval.

No approved product; broader wellness claims outrun the trial record

Human evidence remains limited to two small PK/PD studies and one discontinued Phase II trial. Claims about body composition, recovery, or anti-aging effects in adults are extrapolated from mechanism and the GHRH+secretagogue rationale, not from dedicated ipamorelin outcome trials.
